COVID-19 & Building Resilient Communities Program


The COVID-19 & Building Resilient Communities Program, includes areas of focus outside of the prevention of COVID-19 such as Social Determinants of Health (SDOH) needs. Community based organizations lead engagement strategies for deploying CHWs into high need areas within their respective communities. Social Determinants of Health addressed through this program include: housing & shelter, food insecurity, healthcare, mental health & addictions, employment & income, clothing & household, childcare & parenting, government & legal services, and education.

Central Jersey Family Health Consortium-Regional Grantee

As a regional grantee of a New Jersey Department of Health COVID-19 related initiative, CJFHC funds Community Based Organizations (CBOs) with mini-grants, to develop and implement effective health communication & community engagement strategies to support and strengthen vulnerable populations in the central region, that have been heavily impacted by COVID-19.

The COVID-19 pandemic has disproportionately impacted NJ’s most vulnerable populations.  The purpose of this grant is to support and strengthen community members that have been heavily impacted by COVID-19. This work will be accomplished through the engagement and deployment of CHWs and innovative community programing to address priority populations social determinants of health needs. CHWs support community members by educating on COVID-19 risks and mitigation strategies, connecting individuals to COVID-19 resources and other social supports, and providing ongoing case management. These strategies are to facilitate and build community resilience. 

Colette Lamothe-Galette Community Health Worker (CLG-CHW) Institute

The scope of work is prioritized under the umbrella of the Colette Lamothe-Galette Community Health Worker (CLG-CHW) Institute. The CLG-CHW Institute is an apprenticeship training program for community health workers, critical personnel and allies needed on the front lines of the COVID-19 response. The CLG-CHW Institute was established by the New Jersey Department of Health in honor of Colette Lamothe-Galette, who dedicated her life’s work to eliminating health disparities and addressing health equity. In April 2020, Colette lost her life to COVID-19 complications. To learn more about Colette Lamothe-Galette and her work, click here
