Service Coordinator- Mercer County

We are seeking a Full Time Case Manager/Service Coordinator (SC). The Service Coordinator ensures that children and their families receive the support and service they need, as authorized under the NJ Department of Health Early Intervention Program. The SC assists families with infants/toddlers with special needs (birth to 3 years) by meeting in the home with the family and other practitioners to plan and ensure delivery of EI services.

The qualified candidate must have a Bachelor’s degree or higher from an accredited institution in social work, public health, human services, health related field or education AND at least one year of post graduate documented paid, professional experience working with children (birth to 5 who have developmental delays) and their families in a counseling, supporting or case management role. If the candidate has a social work certification (CSW) or is a licensed social worker (LSW) the one-year experience is waived. Excellent communication skills-both written and oral; proficiency with Microsoft Office, interpersonal and problem solving skills and the ability to travel extensively on a daily basis for community based appointments in Mercer County, including Trenton. Bilingual Community Health Worker- Mercer/ Hunterdon

We are seeking a full time Community Health Worker for our Healthy Women Healthy Families grant. The Community Health Worker conducts community outreach and education, virtual and/or home visits, and case management activities to connect women and their families to health care and supportive maternal and child health community programs and services in their respective county. Other activities include: increase linkages to programs and services through community outreach and enrollment strategies, assistance navigating the healthcare system, appointments, resources and referrals promoting continuity of care, marketing programs and services, conducting education workshops on Maternal Child Health topics and disease prevention, and mapping communities to help locate and support needed services.

Qualified applicants must have H.S. Diploma, BA Preferred and a minimum of 1 year in maternal child health field. Speak and write in Spanish and English. A strong knowledge about Mercer County, New Jersey social services system required and experience relating to the field of public health, specifically maternal child health is strongly preferred. Visiting Nurse Manager – Family Connects

We are seeking a full time Nurse Manager to help launch our new Family Connects Mercer County program in partnership with Trenton Health Team and Capital Health, the first of its kind in New Jersey. The Family Connects model is an evidence-based and successfully demonstrated program that connects parents of newborns to the community resources they need through postpartum nurse home visits offered to families regardless of income or socioeconomic status. Specifically, the goal of Family Connects’ home visits is to support and assess family needs including physical and psychosocial well-being and facilitate referrals and connections to community agencies to support families in the Mercer County area. The Nurse Manager will recruit and supervise a team of nurses who provide home visits to families with newborns. The Nurse Manager will be responsible for the collective success of the clinical team, providing collaborative leadership, support and development for staff, ensuring model fidelity, and assisting enrolled families with clinical expertise and support.

The qualified candidate would be an RN with a B.S. degree, have a current NJ license, and have a minimum of 3 years clinical experience providing care for birthing people during the prenatal, intrapartum, or postpartum period including newborn care. DNP, MSN, MPH, or other advanced degree, as well as supervisory and management expertise preferred. SPOE Service Coordinator

We are looking to hire a full time Service Coordinator for our North Brunswick office. The Service Coordinator will be responsible for referrals and some administrative/management tasks to support the SPOE Referral Manager. This position ensures that children and their families receive the support/service they need, as authorized under the NJ Department of Health Early Intervention grant funded program for infants/toddlers with special needs (birth to 3 years). The Service Coordinator completes the initial phone contacts (no face to face contact) with families via an electronic process. The Service Coordinator must complete referrals on a daily/weekly basis with accuracy and speed.

The qualified candidate must have a Bachelor’s degree or higher from an accredited institution in social work, public health, human services, health related field or education AND at least one year of post graduate documented paid, professional experience working with children (birth to 5 who have developmental delays) and their families in a counseling, supporting or case management role.
